It can take up to three additional business days for a network search. There's no additional charge for orders of 1,200 or more records in the search. For fewer records, a surcharge may be added to the per-email rate. Super-premium email addresses, such as cabinet- or management-level titles at medical organizations, may be priced between 5% and 25% higher than the clinician email pricing shown here, based on quantity and source, and may take additional business days for delivery. Prices are set to account for small changes in quote counts; quoted counts should be considered approximate, as updates may make for more or fewer records in the delivered file, and email addresses do "go dark" regularly. All sales are covered by the CarePrecise Email Deliverability Guarantee. Due to agreements with sources, and our anti-spam pledge, free samples are no longer available. However, an Email Evaluation Package of 500 CP Preferred email addresses, based on your specialty and geographic location criteria, can be ordered for $250, refundable with a full order purchase of 2,500 email addresses or more within 30 days, as long as search criteria has not changed (specialties and geography, etc.). Contact us for an invoice for your choice of quantity (5,000 credits minimum, in blocks of 2,500), and then you can use them up throughout the year, drawing on your credits instead of paying for each separate order – with just one processing fee.

Plus, Credit Pack customers can have CarePrecise keep a suppression file for them, containing all of the email addresses they've purchased from us so they won't be charged more than once for any email address, and even email addresses they've purchased elsewhere. While our other customers pay a minimum of $40 per order with suppression, and may have to keep their own file, sending it to us each time they order email addresses. Credit Pack Details: Minimum Credit Pack order is 2,500 credits. Credit Packs may be ordered in increments of 2,500 credits; i.e., 2,500, 5,000, 7,500, 10,000, 12,500, etc, 1,000,000 credits maximum per order. "One time download" price is not applicable to Credit Pack orders. Minimum credits used per email address order is 500. Credits are good for one year following purchase (two years at the 250,000 Credit Pack level and higher) and are not transferable to the purchase of other CarePrecise products and services, or for refund. If order cannot be filled due to customer's search criteria, no refund or adjustment can be given. CarePrecise offers a "credits credit" of 5% for bounce reports from Mailchimp, Constant Contact and most other other US-based mainstream bulk email transmission vendors when the bounce report covers 10,000 or more sends, calculated on remaining credits in the pack. This is in addition to refunds or credits for invalid emails as covered in the deliverability guarantee.

How to Calculate Your Cost

Instead of buying a Credit Pack, you can just purchase a list of specific kinds of provider in any geographic area you prefer. We can either send you just one email address per practitioner/NPI number- the CP Preferred Email™ address - or you can choose to purchase all of the email addresses we have for each practitioner.

Preferred Email: One email per contact. Receive only the CP Preferred Email address for each practitioner on your list. You will pay the rate as shown above for each email address, plus processing fees as applicable. This is the default.

All Emails: All email addresses. Receive all email addresses we have for each contact. In many cases, we have more than one email address per practitioner - personal, business, practice group, hospital or university residency, and so on. Entrepreneurial physicians, for instance, may have a separate email address for each of their business enterprises. Request this alternative option if you want to assure the highest potential for reaching your targets.

An Email List Based on Your Specific Target Criteria

We can create a custom email list for you based on criteria you request. You can select by of these criteria or nearly any mix of them:

• Specialty/subspecialty (provider taxonomy code)
  About Taxonomy Codes

  Provider Taxonomy Codes (specialty and facility types) are 10-character codes that are self-selected by the provider to designate their classification and specialization. The codes do not identify specific services rendered, but rather, they are related to the provider's education and training for particular specializations. Providers may report up to 15 taxonomies on their NPI record. In CarePrecise datasets, the provider's taxonomy code(s) and licenses associated with those codes are in a table that also indicates the primary taxonomy. The industry-standard Provider Taxonomy Code Set is updated annually in January with new or changed codes. Updates go into effect on April 1. Providers must update their selected taxonomy codes when they change. There are two special taxonomy codes that are used to identify practice groups – 193200000X identifies multi-specialty groups (groups having members with multiple taxonomies), and 193400000X identifies single-specialty groups (groups having members with a single taxonomy); these codes are not found in the counts shown here. Where licenses are required to be reported for a particular Taxonomy, the NPI enumeration system prompts the provider for the license number. Full details on codes may be found here.

An additional processing fee will be added to your quote based on the complexity of filtering, and you can suppress tour existing email addresses or NPI numbers starting at $40, with suppression based on phone, name, address or other customer-submitted data starting at $100, based on complexity. CarePrecise maintains email addresses for hundreds of different kinds of healthcare providers, from physicians to nurses, dentists, and all other types of healthcare prtovider. We have podiatrists (also considered physicians), and chiropractors, along with physician assistants/APN/NP, PT/OT, and all other types of healthcare providers.

What's in the Excel and CSV files we send?

Your file will contain more than just the email address. You'll get the NPI number, practice address, phone(s), primary specialty, credentials and more. Note that CompanyName and Title are for organizations' records only, and will not appear for Individuals' records, unless specifically requested for an add-on charge of $149. CompanyName field is populated for Type 2 (organization) records; we can append company names for Type 1 (individuals) records as an option (below).

Data Format

Your file will be in CSV format for use in all major spreadsheet and database software. If you order one of the relational database formats shown below, your delivery will include relational database formats in two formats with the identical data: CSV and Microsoft Access MDB.

  The Estimated Years in Practice allows inference of approximate age, especially for physicians. It is calculated based on the practitioner's first NPI enumeration date, certification date, and medical school graduation date, where available.

• Add Practitioner's Practice Group and Hospital Affiliations

  CarePrecise tracks clinicians' affiliated practice group(s) and hospital(s), where available.

• All Records

  Our search usually turns up more records that match the submitted criteria than we have email addresses for. This option, if requested with your order, gives you the Excel and CSV files, exactly as in the standard way shown above, but also includes all of the records we find matching your criteria.

• All Data, All Records

  Just as with All Records option above, when our search turns up more records that match the submitted criteria than we have email addresses for, this option not only gives you the Excel and CSV files as in the standard way shown above, including all of the records we find matching your criteria or NPI list, but also a relational database with deeper information on the providers – license, all of the provider's specialties, other names, and more. Approximately 98% of physician records include state licenses. (Does not include cell phones, geocode data, practice group and hospital affiliations, and some other of the premium options.   Orders of 25,000 or more email addresses are eligible for the ADAR Option at no charge if requested with your order.

• Web Address Append

  We can include the web address for a flat append processing fee, if requested with your order. We have web addresses for about 50% of our email database, and how many you'll receive depends on the specific records in your order.

• BestFax™ Verified Fax Numbers Add-On

  Our email lists, by default, include only the fax numbers that are reported by the providers on their NPI records, but many providers do not list a fax number, or deliberately report a non-working fax number to avoid its use by spammers. But using our QoRelate™ advanced data mining technology we are able to acquire many more fax numbers from other than the basic NPI registry data, and we confirm them by multiple testing runs. • Practice Location Geocode Append

  CarePrecise HealthGeo™ offers an optional rooftop latitude and longitude append for the specific practice location, not just for the Zip Code center. To append address geocodes to your email list file, the minimum price is $99 when ordered with an email purchase (up to 1,500 records), priced at $0.05 per record over 1,500. • Cell Phone Numbers

  We can provide the cell phone numbers for healthcare providers of all kinds. Pricing is based on quantity. Cell phone pricing runs between $0.18 for larger orders (10,000 and more) and scaling to $0.50 per number for smaller orders. These can be appended to your email order, or may be ordered separately. There is a minimum order of $250 on cell phone numbers, plus the $90 processing fee, separate from the minimum on email addresses. END USER LICENSE AGREEMENT - Permissible Use of Healthcare Provider Email Addresses

License Agreement (or "License") by and between CarePrecise LLC ("Licensor") and Purchaser Licensee (or "Purchaser" or "Licensee"), collectively the "Parties," relating to the purchase of email addresses and accompanying data and product components ("Email Product"); wherein: Email Product obtained by Purchaser Licensee from Licensor may be use by Licensee without a time limit and as many times as desired, and that it may not be sold, rented, or otherwise shared with or used on behalf of any other party without the separate purchase of a special license agreement explicitly defining permitted sharing. Permissible use is limited to email sent on behalf of the Purchaser's own direct business entity alone, and not on behalf of any third party. Email Product is not covered by the standard Derivative Product tiered licenses unless email address inclusion is explicitly stated in the License. Purchaser agrees to store and treat Licensor data, including Email Product, in the same electronic forms and with the same level of security and confidentiality as it does its own proprietary information, and to limit exposure to only those persons, employees, or contractors directly involved in the business use of the Email Product on behalf of the Purchaser. Purchaser agrees to honor diligently all requests received from Email Product addressees to be removed from future mailings ("opt-out"), and agrees that all email sending and opt-out fulfillment will comply with the U.S. CAN-SPAM Act or 2003 and its successors and international equivalents, regardless of the country and laws from which, under which, and to which email is sent. All Email Product must be purchased with a written request (via email sent to Licensor, or a contact form on the Licensor's website such as this contact form). Licensor retains the right to scrutinize requests to purchase Email Product, including but not limited to blacklist checks and online reviews, and to refuse sale of such to any prospective buyer, and the right to sever relationship with any existing purchaser, at the sole discretion of Licensor. Purchaser agrees that it will cease to use, send to, or store Licensor's Email Product should Licensor discover abuse or use not in accordance with all laws and the CAN SPAM Act of 2003 or its successors or international equivalents. Purchaser agrees that it will not use Email Product for any illegal purpose, including but not limited to "phishing" to acquire personally identifiable information or to cause damage to any recipient or carrier's systems or business processes. Purchaser understands that Licensor disavows any responsibility for its use of Licensor data including any Email Product or portion thereof, and that the laws of the United States of America (USA) and legal venue governing this License Agreement shall be Tulsa, Oklahoma, USA, and that the limit of Licensor's total aggregate liability in all actions related to these Parties shall be limited to US$100.00. Email Product is offered without warranty except for or beyond the terms of the Email Delivery Guarantee defined at this link. This License Agreement supersedes any representations, written or verbal, at the time of purchase, and should be documented as of the date of purchase with the understanding that subsequent purchases may be subject to an updated License Agreement. Purchase of Licensor Email Product affirms the Purchaser's understanding and acceptance of and agreement with these terms and limitations, and affirms that Purchaser and Licensor have entered into a binding "purposeful agreement" as understood in contract law to apply to agreements forged in writing via email.
